Due to strict insurance guidelines, Artemis Intraoperative Monitoring has implemented an insurance verification process requiring prior authorization. Our office will be verifying insurance coverage prior to surgery, when possible. In order to verify coverage, please complete the insurance info and patient demographics on our form.

When you call, fax, or email our scheduling department, please be prepared to give the following information to the scheduler:
Date of Service
Time of Surgery
Patient Date of Birth
Patient Social Security Number (if available)
Type of Surgery / Levels if available
Approximate Duration of Surgery
Patient Insurance Information
We will also send you our scheduling forms to your office to be filled out by your staff.
Please also be aware that our scheduling department will also confirm the case the afternoon prior to the surgery date. If there is a particular department or person that you would like us to confirm with, please let us know.
